Hard copies of this document are available in the ASB office.
All attendees, even if they don't attend our school, must download and submit this document. It must be signed by the following parties:
- Parent or guardian (students who are 18 may sign their own)
- Library
- Accounting office
This form is due at the time of ticket purchase.
