Kealakehe High School, in partnership with the Nakuwauna Foundation, will be holding a FREE sports physical clinic on June 19th from 9:00am to 1:00pm in our school's gymnasium. Sports physical forms are available to print below or to be picked up in the main administration office at Kealakehe High. The forms must be filled out and signed by a parent/guardian prior to bringing them for the sports physical. The completed physical forms (after the doctor signs) are good for 1 year, allowing students to participate in their chosen sports for the entire school year.
Help stock Kealakehe High's Kōkua Closet with toiletries. Bring an unopened item or few when you come to receive your free sports physical.
